I hereby acknowledge that I have voluntarily chosen to participate in activities, programs, and/or use the facilities and equipment provided by UFC Gym White Plains. I understand that these activities involve inherent risks and dangers, including but not limited to physical injury, death, and/or property damage in consideration for being permitted to participate in these activities, I hereby voluntarily release, waive, discharge, and covenant not to sue UFC Gym White Plains, its owners, employees, agents, contractors, and representatives, for any and all claims, demands, damages, and causes of action that may arise from or be connected in any way to my participation in these activities, including any claims or damages caused by the negligence of UFC Gym White Plains, its owners, employees, agents, contractors, and representatives.I further agree to indemnify and hold harmless UFC Gym White Plains, its owners, employees, agents, contractors, and representatives, from any and all claims, damages, costs, and expenses, including reasonable attorneys' fees, arising from my participation in these activities.I understand and acknowledge that this waiver and release of liability applies to any and all injuries, damages, or losses that I may sustain while on the premises of UFC Gym White Plains, whether or not such injuries, damages, or losses are caused by the negligence of UFC Gym White Plains, its owners, employees, agents, contractors, and representatives. I also grant permission to UFC Gym White Plains to contact me for the purpose of soliciting feedback, offering promotions, and providing updates about the gym and its services. I further grant permission to UFC Gym White Plains to use any photographs, video recordings, or other media of me captured during my participation in these activities for promotional or advertising purposes without any compensation to me, and without any further authorization or approval from me. certify that I have read and fully understand this waiver and release of liability, and that I am signing it voluntarily and without any inducement, coercion, or undue influence.
